Dr. Laura Dean: Today's case is that of a 26-year-old man who was transferred to the emergency department
(ED) by ambulance from the scene of a fall from a fourth-story window. The patient
had been drinking alcohol that evening and the fall was witnessed, however, no additional
information regarding the context of the fall was available. On the arrival of emergency
medical services (EMS), the patient was unresponsive with a systolic blood pressure
in the 80s and a heart rate in the 110s. His breathing was agonal, and he was intubated
in the field without medication. En route, he received 500 cc of normal saline.
